QuitFax Referral System
Through the QuitFax Referral System, healthcare providers can help connect their tobacco-using patients with local cessation services.
Many Arizona healthcare providers already encourage their patients who use tobacco to call the ASHline for help. However, the proactive referral system is more effective because it guarantees at least an initial contact with the patient.
Here's how the system works:
- A healthcare provider (HP) intervenes with a tobacco-using client and assesses his/her readiness to quit.
- If the client is ready to quit:
- HP fills out the QuitFax Referral Form and gives the client a Script to Quit.
- HP faxes the referral to the Arizona Smokers’ Helpline.
- Ashline does the following:
- assigns a counselor to the client;
- records the referral for reporting purposes; and
-
faxes a confirmation to the HP.
- An Ashline counselor calls the client. The counselor assesses which service is appropriate for the client (classes or telephone counseling), and makes a referral.
- For reporting purposes, the counselor records the services provided.
- Ashline reports the outcome of the referral back to the HP (including if the counselor could not reach the client).
- If the client is not ready to quit:
- HP gives the client a “Ready Now?” wallet card and a magnet.
- HP reassesses the client during the next office visit.
