QuitFax Referral System

Through the QuitFax Referral System, healthcare providers can help connect their tobacco-using patients with effective treatment to quit tobacco use.

Here's how the system works:

• Healthcare professional fills out QuitFax form with Client and faxes to ASHLine
• ASHLine returns a fax confirmation report.
• Within 24 hours ASHLine makes first attemtp to contact Client.
• Up to 5 attempts are made within 10 days to reach Client.
• IF REACHED, Client is offered telephone-based quit tobacco coaching.
• Healthcare professional is notified of Client decision to accept services or self-help information by fax.
• IF NOT REACHED, Healthcare professional will be notified so follow-up can take place with client.
• Healthcare professional will receive quarterly newsletter containing the status of all clients referred that quarter.
